Excavation/Lifting/Crane
Procedures

Excavation

    • Excavation Permit shall be required prior to the excavation work.
    • Where excavation on the existing route is implemented, the cable detector shall be used to detect the cables below if any, and the necessary precaution will be taken.
    • In event that an electrical cable or other buried service is exposed, all works shall halt immediately. Under no circumstances shall work resume until so advised by an appropriate authority of Contractor.
    • Machinery shall be positioned and operated in a manner that will not endanger any person involved in the excavation work.
    • Excavation with a depth of more than 1.5m shall be shored, sheet piles or sloped.
    • All excavation area shall be barricaded with warning signboards and provided with appropriate warning lights during nights for all area outside the Process Area.
    • Gas cylinders shall not be positioned in any excavations.
    • In case of damage to the existing cable, supervisor shall immediately inform Contractor's personnel.

Lifting Supervisor

    • Must be provided / available for all lifting operation.
    • Must put on luminous vest / and band for identification during lifting  operations.
    • Being the competent person, i.e. after  attended  courses  conducted  by  the  relevant  local  government,  Will  be  responsible  for  the  correct  use  of  all  lifting  gears,   appliances  to  ensure  the  stability  of  cranes  with  proper  support  on  solid  flat  ground  with  the  use  of  timber  mattress  or  steel  plate,  out-riggers  to  be  fully  extended,  and  to  check angle  of  lift,  overloading,  provide  rag  line.  The proper hand signal should be given to the crane operator to ensure all lifting works to be carried out safely.

 Crane

    • The On-Site GM Industries supervisors shall instruct the lifting operation.
    • Cranes coming to the site shall possess and display a valid certificate.
    • All works involving the use of a crane shall be property planned in advance, and a rigging study executed to ensure that:
      • Hot work permit must be applied where applicable.
      • The crane is capable of lifting the load, given the known working radius, boom length, weight of the load.  Etc.
      • The rotation zone, assuring the boom is not restricted.
      • Suitable matting or plates are available to protect underground services and paving.
      • All slings, shackles, hooks, etc., with relevant certificate and of the correct rating are available and in good condition.
      • The angle between sling legs shall not, under any circumstances exceed 120 deg., as this doubles the tension on each leg, thereby reducing the lifting capacity of the sling by 50%.
      • Only one qualified person shall give instructions to the crane operator.
      • For mobile cranes, outriggers shall be used for each lift, regardless of the size, and shall always be fully extended.
      • A trial lift shall be performed, raising the load a short distance above its pick-up point, to check the stability of the crane, and the efficiency of the brakes.
      • The load shall, at all times, be kept as close to the ground as possible.
      • Whenever possible, and after lifting the load from its pick-up point, the radius of the lift should be shortened to increase the crane's lifting capacity and stability.
      • Wire slings and chains under direct tension during the lift shall not be extended artificially, nor be kinked or twisted.
      • Shackles shall be complete with the original pin, and under no circumstances shall be a substitute bolt fitted.
      • At no time whilst the crane's machinery is running, shall the operator leave the cab and with the load suspended.
      • At no time whilst a load is suspended shall personnel perform any works standing directly beneath.
      • Particular consideration to the presence of low-level structures such as pipe bridges shall be given, where crane is to move about the site.
      • Consideration shall be given to prevailing weather conditions, and where a greater then normal risk exists, the lift shall be postponed until more favorable conditions develop.
      • When reversing the crane, a lookout man must be provided.
